How we get the information and why we have it
Most of the personal information we process is provided to us directly by you for one of the following reasons:

We process personal information to enable us to provide health services to our patients, to maintain our accounts and records, promote our services.
We also receive personal information indirectly, from the following sources in the following scenarios:

Initial enquiries are received through telephone calls, website enquiries, emails and social worker requests to quote for therapeutic work.

Referrals can be received from a number of sources including but not limited to Parents, School, Social Workers and Norfolk County Council.

No therapeutic work is undertaken without the consent of those with Parental Responsibility (PR).
Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for processing this information are:

What we do with the information we have
Information provided is used ONLY in relation to the specific therapy programme requested and the administration of delivering therapy. Contact details are held securely and for the sole purpose of contacting you about your child’s therapy.
Under no circumstances will any personal details be shared with any third party unless legally obliged to do so. Assessments and Reports will be shared if there is reasonable cause to do so in the therapeutic interests of the client, and only where explicit consent is provided. In such cases the document will be sent directly and securely to the approved recipient.
How we store your information
Client information is securely stored in a locked filing cabinet.
For the protection of the therapist and client the child’s personal details and private therapeutic information will be retained for the life of the therapist, after which it will be destroyed.
Website statement
www.childplaytherapynorwich.co.uk is hosted on the Wix.com platform. Wix.com provides the online platform to share information about the types of therapeutic services I offer.
Wix Forms also provides the message facility on the contact page and sends the inputted information to karen.hammond03@gmail.com.
